Basically it is correct, however, there are two things which may be a little bit confusing:
1. I'd separate the verb 'to be' from the rest of the past simple, as your forms may seem that you are mixing past simple with past continuous.
2. I wouldn't use 'used to' (past habits) as an example of past simple as it is usually introduced as a separate grammar and actually it does not follow the form you have so nicely described (there is not -ed form is there?)
